DUI -- Drug and Alcohol Evaluations

If you have recently received a DUI, one of the first things you are going to need is a Drug and Alcohol Evaluation from a State and County approved service provider. Without a Drug and Alcohol Evaluation, the Court cannot grant you a Driving Permit (Monitored Device Driving Permit or M.D.D.P) during the term of your Statutory Summary Suspension.

In order to get a Drug and Alcohol Evaluation, you will need to get a copy of your Driver's License Record (also called a Driver's Abstract) from the Secretary of State. You can get a copy of your Driver's License Record at the local office of the Department of Motor Vehicles (DMV) and you must tell them you want a copy of your record for "Court Purposes." Only you can get a "Court Purposes" Driving Record from the Secretary of State and it currently costs 12 dollars.

You will also need to bring a copy of the police officer's Sworn Report to the Evaluation which indicates whether or not you submitted to a Breath test or other chemical testing and the results of that testing.

At the Drug and Alcohol Evaluation, you will be asked questions about your past drug and alcohol use. Be aware that they are looking for signs and symptoms of Drug or Alcohol dependence or abuse. Some of the most significant symptoms are Loss of Control, Blackouts, and trouble with the law because of drug or alcohol useage.

Certain Counties require you obtain an Drug and Alcohol Evaluation from one specific service provider. For example, Cook County requires you use Central States Institute for the evaluation and Dupage County requires you to use their own Psychological Services Department. Since it is important for most people charged with DUI to get back on the road as quickly as possible, you should schedule an appointment for an evaluation as soon as possible so there is no delay in getting the Court to issue you a Driving Permit.
